What is wrong with my golf swing?

Here's a few clips of me hitting a 7 iron. Sorry the quality isn't the greatest.
The biggest problem I feel that I'm having is I'm coming over the top, or swinging out to in. I can't tell if it's caused by my backswing, or something else. But I just noticed that I pull a lot of shots and also hit a lot of pull-slices with my driver.
I just feel like no matter how hard I try, I can't swing in to out, and I can't figure out what's causing it. Any help would be appreciated.
